Two other (very different) suggestions could be Bourbon Street jazz club on Leidsekruisstraat (not smoker's friendly IIRC, but nice little place with her good music just around the corner from Easy Times where you can go to smoke) and Kashmir Lounge (which has some really nice live dj sets and such...

Jazz Cafe Alto near Leidseplein Maloe Melo, short walk West from Leidseplein. Rock and blues, loud AF in that back room. Gollems Proeflokaal on Overtoom, jazz on Saturday evenings but get there way early. Waterhole, just off Leidseplein, mostly rock, afternoon happy hour for 3.50 pints 14:00-19:00....

Been a very long time (18 years ish) but the weekend of the 20th and a few days either side I will be submerging myself into Amsterdam culture.

Anyone around over the same weekend say hi.

Any nightclubs come recomended, also interested in live music bars/gig venues.
