If I or anyone I know was going to use those toilets I'd be happier knowing that their dirty needles aren't lying about.

One area where I think Vancouver has been ahead of the curve is in harm reduction. We had to do this because we had a terrible IV drugs use problem and the HIV and Hep C rates amongst that population was growing at an alarming rate!
We have had a needle exchange program here for many years. It has certainly slowed down the HIV and Hep C transmission rates amongst the IV-drug user population, and, to some extent, among the general population, as fewer needles are carelessly tossed away. Still, at many many playgrounds, you would want to do a sweep of the grounds before you let your youngsters play there.
We have had a legal injection site (INSITE), here, for several years and it has saved many lives. The junkies are supplied with a kit, so they safely prepare thier fix before injecting. There are medical staff at INSITE and they can get help if the users OD. Also, if the junkies want to get clean they can help to arrange detox and other programs.
We also have a trial legal heroin project, but it is limited to a small number of users, who were chosen by certain criteria, so that the data could be scientifically analysed.
Unfortuantely the federal Tories want to shut down these programs, no matter what the science says, because they are f*cking stupid socially conservative morons!
