After reading this thread, late last night, I dug out my old copies of Mellow Pages. I think one of the copies I have is the same one you refer to. Great nostalgia trip. I also did a search on Google and found that Amazon are offering used copies of MP for $237.72!!
Cheers
I know about Amazon  £ 200  , I got my copy off there for £8ish with postage  :)

I would love to see pics from this era, my first trip was 1992 for a full month! ISo much has changed in my trips through the years that I'd love to see pics of the old long gone and renamed shops..Pink Floyd

I agree with Spidergawd, if you can find someone that reads Dutch, the archives are probably going to be your best bet for finding additional information to the already useful facts provided from the other posts in this thread. I've used municipal and national archives in Mexico and the US for some historical projects of my own and they're almost always the best source of information. Another possibility for finding out the info on early coffeeshops might be online newspaper archives. Many of these have collections that go back decades. Here is a short list of Dutch newspaper collections that have online archives dating back to at least the 1970s and are free to consult. Unfortunately, I can't read Dutch, otherwise I would look up the information you're seeking. The search engines in most of these makes it really easy to find.
