Personally I think "medium well done" only exists in a fantasyland conjured up by the marketing arm of the beef council.
Edt: I like my steak medium btw. I don't mind a little blood, but it better be a hot pink!!

if its a good steak i like it medium rare. if its lesser quality or im not sure about it..like at a sketchy resturant i'll go medium or on the rare occasion medium well. not sure if iv ever done well done, gets to tough at that stage.
for me it also has to be cooked on some sort of grill. not necesarily on open flame(although this is prefered.)broiling is also ok. basically anything but cooked in a frying pan is cool with me. and you gotta let the steak rest!! for a few mins, makes a diff.

since returning from argentina, i havnt had a single steak in the UK. i 100% believe that no where, no where will do one any near as good as you get out there... it was like bloody salty meaty butter... but better! so im not wasting my money any more.

I must agree with c-s-s on this it's practically impossible to get a good steak in this country, 1 of the best I ever ate was from a truck stop diner in Canada seriously it was like bloody butter(must have beenbthe same chef CSS) simply melted away in your mouth, although there was a really good Argentinian steak house near where I lived in Germany too one of those steak on a hot slate places 
I've been told that Miller & Carter are pretty decent here in the UK, we have got plans to try it in a couple of weeks.
I get really good quality meat from my local butchers, it always melts in your mouth even when I overcook it (which does happen occasionaly).
