Help with buying e-ticket for NS train.

Hey, I was trying to but an e-ticket on this page https://www.ns.nl/eticket/ticket?language=en
And what does it means with "surname prefix and surname" ?

I have google it but without any good results :(
please somebody who can explain for me....

I think it's Mr,Mrs or Miss.

Prefix is a word added to another word to change the meaning.

Nice thanks...
So for example my surname is Hernandez , and given name is Carlos.

They want initials and surname prefix and surname...

Surname prefix = Mr
Surname = Hernandez
Initials = CH


I saw something about Mc and Mac ....
From Ireland..
